Simply call 1-844-269-1292 toll free and a friendly representative will assist you. Once you receive your Certificate of Insurance in the mail, you can request additional coverage of up to $300,000.00 for you. You can request coverage for your family as well.
How much additional coverage may I choose?
You alone can decide how much coverage you need. This AD&D Insurance has four coverage levels from $50,000.00 to $300,000.00 all available at group rates. You may want to consult a trusted advisor as to how much coverage is right for you.
Can I get insurance for my spouse and family, too?
Spouses and unmarried dependent children may be insured under the family plan. If you choose the family plan, spouses are insured for 60% of your additional coverage amount. Dependent children are each insured for 20%. If you are a single parent, each of your dependent children will be insured for 25% of your additional coverage.1
Do I have to take a medical exam or answer a lot of health questions?
No medical exams, blood tests, or health questions are required to get and keep this coverage.

Federal Insurance Company, a Chubb company is the underwriter.
Are there any additional benefits?
You automatically receive the following benefits when you select added coverage:
Common Carrier - this benefit pays twice the elected additional coverage if your loss occurs due to a covered accident while you are in, entering, or exiting a common carrier.
Education Expense - if you suffer an accidental death, this benefit reimburses education expenses (up to the maximum shown in the Insurance Coverage Document) for each dependent child who is enrolled as a full-time student in an institution of higher learning.
Inflation Protection - this benefit increases the benefit by 2.5% for each year that your coverage is in-force until it reaches 125% of the original amount.
Enhanced In-Hospital - this benefit pays $100 per day for each day of hospitalization, to a maximum of 365 days.

What is a "Common Carrier"?
Common Carrier means any motorized land, water or air conveyance licensed for the transportation of passengers for hire and operated by an employee or an individual under contract. For example, commercial airlines, trains, buses, cabs, ferries. Common Carrier does not include any conveyance used for recreational activities.
What coverage is provided by this insurance?
This insurance provides Accidental Death and Dismemberment benefits in the event of a death or loss of life, limb, sight, speech, or hearing as the result of a covered accident.
What is not covered:
Insurance does not apply to any Accident, Accidental Bodily Injury or Loss when the United States of America has imposed any trades sanctions prohibiting the insurance, or there is any other legal prohibition against providing the insurance. In addition this insurance does not apply to any Accident, Accidental Bodily Injury or Loss caused by or resulting from, directly or indirectly: 1) Loss occurring while an Insured Person riding as a passenger in, entering, or exiting any aircraft while acting or training as a pilot or crew member. This exclusion shall not apply to any passengers who temporarily perform pilot or crew functions in a life threatening emergency; 2) an Insured Person's emotional trauma, mental or physical illness, disease, pregnancy, childbirth or miscarriage, bacterial or viral infection, bodily malfunctions or medical or surgical treatment thereof. This exclusion does not apply to an Insured Person's bacterial infection caused by an Accident or by Accidental consumption of a substance contaminated by bacteria; 3) an Insured Person being incarcerated after conviction; 4) an Insured Person's commission or attempted commission of any illegal act including but not limited to any felony; (In MN: an Insured Person's commission or attempted commission of a felony or being engaged in an illegal occupation); 5) an Insured Person being intoxicated, at the time of an Accident. (In MN: an Insured Person being voluntarily intoxicated while driving a vehicle at the time of an Accident); Intoxication is defined by the laws of the jurisdiction where such Accident occurs; 6) an Insured Person being under the influence of any narcotic or other controlled substance at the time of an Accident. This exclusion does not apply if any narcotic or other controlled substance is taken and used as prescribed by a Physician; (In MN: an Insured Person being under the influence of any narcotics unless administered on the advice of a Physician); 7) an Insured Person participating in military action while in active military service with the armed forces of any country or established international authority. However, this exclusion does not apply to the first sixty (60) consecutive days of active military service with the armed forces of any country or established international authority; 8) an Insured Person's suicide, attempted suicide or intentionally self-inflicted injury; 9) a declared or undeclared War.
Who is covered under the Family Plan?
The Family Plan would provide coverage for yourself, your spouse and any eligible dependent children that you may have. Please refer to your Insurance Coverage Document for more detailed information.
Is there an age cutoff?
There is no termination age; however, all coverage is reduced by 50% at age 70 and older.
Does this policy have a cash value?
No. There is no cash value for this insurance.
What if I have other insurance?
This insurance will pay all benefits due to a covered accident regardless of any other insurance that you may have.
Who can I name as my beneficiary?
Name anyone or as many beneficiaries as you would like.
Are on-the-job accidents covered?
Yes. Your coverage is effective 24 hours a day, anywhere in the world.

How will I be billed for my coverage?
There is no charge for the complimentary coverage. AltaOne Federal Credit Union pays it in full for you. Additional coverage you select will be conveniently billed to the account provided at the time of enrollment.
What day does the premium come out of my account?
About the 5th day of your billing cycle due date.
How do I file a claim?
You can receive and complete our Insurance Claim Forms by calling our Claims Department at 1-800-252-2148. A representative would be glad to assist you in filing a claim.
How long does it take to process my claim?
For all benefits payable under the policy, the insurer will pay the Insured Person or beneficiary the applicable Benefit Amount within 60 days of receiving complete Proof of Loss.
Why do you need my hospital records to process a claim?
These records are required by the insurance company to verify that the primary cause of death/dismemberment was due to a covered accident because this is Accidental Death and Dismemberment insurance.
What if I can't get an accident report?
Enclose name or business card of the investigating officer with your claims forms. The insurance company can then get in contact with the police directly for additional information that may be required in adjudicating your claim.
